Case Management Services

Living with health problems and managing them can be hard. Community First Health Plans case management team can help. Case management is when a trained registered nurse works with you and your Primary Care Provider (PCP) or other healthcare providers, to help you understand your condition and how to better take care of yourself. They will also provide you with resources that can help you get the best care possible.

We also offer a Complex Case Management program for members with difficult health problems such as cancer, asthma, diabetes, behavioral health or substance abuse, who need extra help with their special healthcare needs. The nurse will work with you to create a self-management plan to help you learn more about your condition to keep you as healthy as possible.

The case management team will help you:

  • Identify your needs
  • Coordinate care with your doctors and therapists
  • Develop a plan for your care
  • Assist in ordering special equipment and supplies
  • Coordinate behavioral health services
  • Initiate home health care
  • Assist with access to community programs
  • Evaluate additional needs
  • Track your progress overtime or changes in your condition

This program is voluntary and is offered at no cost to you. You can choose to be removed from the program at any time.

Our Case Management Department is committed to working with you, your family members, your doctors, and other members of your health care team, to improve your health and access the services you need.

There are many ways you can start a program. One way to be referred is by your doctor. You also can self-refer to the program by the Member Services number on the back of your CFHP card.
