Medina
Wilson
If your office is not located or you do not service one of the above counties, we appreciate your interest in establishing your status as a participating network provider. However, we are unable to contract with you at this time.
If your office is located or you do service one of the above counties, please complete the Letter of Interest form.
The completed Letter of Interest is submitted to the Network Access Review Committee. The committee performs and reviews a geographic accessibility study of current providers of similar specialty and benefit program requirements.
If your request is approved, your Provider Relations Representative will contact you with information regarding Contract and Credentialing requirements.
If your request is not approved, you will receive a letter of disposition regarding your request.
