Community First Health Plans offers a full range of health plan products, including STAR Medicaid. Our network doctors and providers will help you get the care you need. We are the only non-profit, locally owned health plan available to you. If you have questions, call us. We can help you.
What is Community First's Service Area?
Community First's service area includes all of Atascosa, Bexar, Comal, Guadalupe, Kendall, Medina and Wilson counties.

As a STAR member, you have access to the following extra STAR benefits including:

What is the Member Portal?
Click on the Member Portal link in the orange Member Login box to register. This is a secure website. You can request an ID card, ask to change your PCP, file a complaint, or ask a question. If you are more comfortable talking to a Member Services Advocate, just call. We are here to help you!
Your Primary Care Provider (PCP).
Your Primary Care Provider (PCP) is your own doctor or clinic who will provide or arrange your medical care. You can look up your PCP or any specialist in the Community First network on this web site using the "Find a Physician" link in the Quick Links box. In addition, you may
download a pdf document with a directory of providers and additional helpful information.
What is “Find a Physician?
Click on “Find a Physician” in the Quick Links box to list all physicians in the CFHP network.
What if I want to change my PCP?
- See the Community First STAR Provider Directory for a list of PCPs and clinics with offices near you.
- Or, use the “Find a Physician” search link at the top of this page.
Then, you can call a Member Services Advocate or you can request a PCP change through the Member Portal.
What should I do if I have a complaint?
Please call us. A Community First Member Services Advocate can help you. Most of the time, we can help you right away or at the most within a few days. We can help you file a complaint. You can also find our complaint form on this website in the Member Portal. We will let you know we received your complaint, by mail, within five days and send you a decision within 30 days.
At any time, you may file a complaint directly with the Texas Health and Human Services Commission (HHSC) by calling the Medicaid Hotline at 1-800-252-8263.
