Community First Health Plans recognizes that potential members may have concerns about changing health plans or the need to change providers due to a provider terminating his/her contract with Community First. Either of these situations may affect the ongoing medical care they are receiving.

“Transition of Care” involves any ongoing, active course of treatment, the current phase of which the interruption of service might possibly cause undue harm to the member. For commercial members, this information is shared with employer representatives during the marketing and enrollment periods. If you believe the transition of care policy may apply to you, please fill out the attached Request for “Continuity of Care Coverage” form and submit it to Community First as soon as possible prior to your effective date.
