Order ID Cards

Community First Health Plan members may request an ID card through our secure online member portal. Members may access this secure portal 24 hours a day, 7 days a week.

To request an ID card, members may click on the orange Login box at our home page or the Member page to register and order an ID card. If a member has any questions about their ID card, they may contact our Member Services department at (210) 358-6070 or toll-free at (800) 434-2347.
