How do I make Eligibility changes such as Adds and Terms?
Send all Eligibility requests to you designated Account Services Coordinator via e-mail or fax. To add an employee, send a completed Enrollment form with in 5 days of the effective date. To term, send first and last name, brief reason for termination, termination effective date, and last four of the Social Security Number.
When must I notify CFHP of a termination?
Notification must be given as soon as possible. According to Senate Bill 51, the employee will have coverage through the end of the month in the month when notification was given. If an employee terms at the end of the month, the employer has 5 days into the next month to notify CFHP of the term.
How do I make corrections to the bill?
Email your ASC with your specific concerns. They will need to know the employee's name, date of birth, and effective date/term date in order to make corrections to your bill. You may also request a copy of your bill through your ASC.
How do I update addresses, phone numbers, and other personal information on an employee's account?
Send all updates and changes to your Account Services Coordinator using the CFHP Enrollment/Change Form.
How do employees register for Above Health?
Go to the CFHP website where you will find a link to the Above Health website. Click on Register if a member. Read the registration terms and conditions. Clink on "Yes I agree with the statement." Enter your personal information including your last name, first name, birth date, and member number (this is the A1234567890 number on your Community First ID card.) How can my employees view their EOB's online?
EOB's can be viewed on the Above Health website after registering as a member.
How do I reorder an ID card?
Instruct the employee who needs the replacement card to call Member Services at 210-358-6070 to request a new card. The replacement card will be mailed directly to the employee's home address. You can also refer the member to the Above Health website.
How do I view my renewal rates for my groups?
Contact your Account Services Coordinator to receive renewal rates and alternative plan designs. You may also view the renewal rates on the CFHP website under Renewals.
